What to Expect During Your Stay

We are thrilled you are considering a Pavilion care center. Please use these resources to assist you in your search.

Care Team

Upon admission, residents are evaluated by an interdisciplinary team, including a certified rehabilitation and nursing staff trained to meet special needs and create a personalized care plan. Our team monitors care and reports your care plan to the physician. Our specially trained physical, occupational, and speech rehabilitation therapists work closely with Physicians to create a program of personalized care to support the very best outcomes and to return you to an active life at home as quickly as possible. For rehabilitation patients, home visits are also preformed to ensure a successful transition home.

Your care team may include:

  • Consulting physicians and physiatrist
  • Registered and licensed nurses and nursing aides
  • Physical and occupational therapists
  • Speech and language pathologists
  • Nurse practitioner
  • Social worker and discharge planner
  • Dietitian
  • Dedicated case manager

What to Bring

Upon admission, please bring the following items with you, if applicable:

  • Social Security Card
  • Medicare Card
  • Insurance Card(s)
  • Medicaid Card
  • Pharmacy Card
  • Driver’s License/Photo ID
  • Power of Attorney for Healthcare
  • Power of Attorney for Finance
  • DNR/Advance Directive
  • Living Will

Please label all pieces of clothing with the guest’s name using a black laundry marker. Suggested clothing includes:

  • Comfortable slacks, shorts, sweatpants
  • Comfortable blouses or shirts
  • Sweater
  • Tennis shoes or sensible shoes
  • Nightgown or pajamas
  • Undergarments
  • Toiletries and personal items

You are welcome to bring items to decorate the room such as pictures or flowers. We encourage residents to refrain from keeping jewelry, valuables, and money in their room. Please speak with the Administrator about secure storage of these items.

Insurance Options

Pavilion of Logan Square accepts various insurance plans, ensuring financial coverage for comprehensive healthcare services, including rehabilitation, nursing care, and specialized treatments.
